Information to claim for OVERSEAS TRAVEL INSURANCE,
”OMOTENASHI ANSHIN”PLAN
You can claim your Medical Expenses in Japan (the expenses you have paid for Doctor’s Fee,
Prescription, Medical certificate, Interpreter, Transportation, etc.), according to the guide as follows.
This process is not required, if you have not paid noteworthy cost by receiving
“Cashless Medical Service”. You may contact us , if you want to claim Accident Death, Rescuer’s
Expenses, or Personal Liability, or when you have any question.

STEP 1Document Submission〈CLAIMANT〉
（1）The Necessary Documents
１） Overseas Travel Insurance Claim Form(For Injury or Sickness)
２）Accident Report(For Injury)
It is no need if the section ⑥ of the claim form was filled in.
３）Medical Certification（For Injury or Sickness）
You must submit the original. No copy is accepted. It is no need if the section ⑤ of the
claim form is filled by your attending doctor, or if you have received the
“Cashless Medical Service”.
４）Receipt(s) and Detailed Statement(s) of the cost you have paid (For Injury or Sickness)
You must submit the original. No copy is accepted.
５）Passport Copies (For Injury or Sickness)
Please submit the pages of Photo ID and the immigration stamps of JAPAN.
（2）Please send the necessary documents along with the duly filled claim form to the address
shown as below. Please refer to the section (2) for the necessary documents.

（1）After receiving the documents, we will confirm the documents and determine
the insurance payment propriety and the amount.
（2）If necessary, we may ask the additional documents, or your cooperation for the research, etc.
STEP 3Payment〈JI Accident & Fire insurance Co., Ltd 〉
（1）We will make the payment according to your instructions in your Claim Form.
（2）We will contact to you if your claim(s) is denied.
Main Exclusions
We cannot pay the items shown as follows:
＜Medical Expenses for Injury / Sickness＞
- ・Dental disease
- ・Injury before arriving Japan
- ・Routine checkup, vaccinations or immunizing injections
- ・Chiropractic, Acupuncture, Moxa cautery without the medical doctor’s instruction
- ・Injury while driving without an effective license in Japan, or drunk driving
- ・Injury in struggle act such as a fight
- ・Medical expenses beyond 180days from the date on which the accident occurred or medical
consultation was first received for the disease - ・Medical expenses outside of Japan
※Please contact us in the case of Sickness before arriving Japan, or Sickness related to
pregnancy, childbirth, premature birth or miscarriage.
